To open and fill out a ".pdf" version of the Reservation Form online, just click here and complete each block. Then TAB to each new entry block and type in the data as indicated. There is a "START OVER" button at the top right-hand side of the form itself should you need to clear all entries and begin again.

Then print out the completed form, sign it, and mail it to the address shown at the end of the form along with your deposit check of $300 per person.

If you prefer to print out the blank form first and then hand enter the data, you can turn off the "Highlight Fields" toggle button at the top right-hand corner of the screen by clicking on it. Then print it, hand enter the detail, sign and date it, and mail it in with your deposit check.
